NBDHE Candidate Guide: test specifications and changes

The NBDHE Candidate Guide is the official rulebook for the National Board Dental Hygiene Examination (NBDHE), from the Joint Commission on National Dental Examinations (JCNDE). You have to confirm you’ve read it when you apply. The 2026 edition is marked updated September 17, 2026. It prints two sets of test specifications, because an update was due in November 2026.

  • 350Questions under both sets of specifications
  • 12 to 15Patient cases behind 150 of them
  • November 1, 2026Updated specifications scheduled

What’s in the NBDHE Candidate Guide?

It’s the rulebook as well as the syllabus. Inside you’ll find:

  • Eligibility routes and the documents each one needs
  • Fees, the six-month testing window and the retake rules
  • Test center procedures, ID rules and banned items
  • Testing accommodations and how to ask for them
  • How scoring, results reports and audits work
  • Rules of conduct, penalties, appeals and the agreement to arbitrate

Changes can come after it’s published. The JCNDE posts updated PDFs on its NBDHE page, and you’re bound by the policies in effect on your test date.

NBDHE test specifications, before and after the update

The guide gives both sets, in items. The discipline-based component stays at 200 and the case-based component at 150. The research area is renamed Research Principles and Community Health.

The JCNDE’s updates list, dated October 22, 2026, scheduled the updated specifications for November 1, 2026. It says they aren’t expected to change how you prepare much, but they add clarity and refinements. Lines that appear for the first time include local anesthesia, occlusal evaluation, emerging technologies, and documentation and risk management. Study from the specifications that go with your test date.

How the two sessions are timed

Here’s the schedule from the 2026 guide. The tutorial, breaks and survey are all optional.

PartItemsMinutes
Tutorial015
Session one, discipline-based200210
Break between sessions030
Session two, patient cases150240
Post-exam survey015

Each session also has an optional 15-minute break halfway, and the session timer carries on with whatever time is left. That’s 7 hours 30 minutes of answering time and 9 hours in all. The timer pauses while a question loads. Any other break counts as unscheduled: no locker, phone, food, drink or notes, and you can’t leave the center.

Question formats the guide shows

Every question is multiple choice, with three to five options and one correct or best answer. The guide describes these types:

  • Completion: the stem starts a sentence and the options finish it.
  • Question: the stem asks directly about a problem or situation.
  • Negative: a capitalized EXCEPT or NOT in the stem.
  • Testlets: a short case or problem with a set of questions, used for research and community health.
  • Patient box: case details beside the question. An empty area means the information is unknown or not available.
  • Dental charts: some items include them, and the guide shows samples.

Teeth are numbered with the Universal/National system: 1 to 32 for permanent teeth and A to T for primary teeth.

Guidelines the questions follow

The updates list says which clinical guidelines the questions reflect. For the NBDHE it names:

  • The American Academy of Periodontology’s 2017 classification of periodontal and peri-implant diseases
  • The American Heart Association’s 2021 statement on antibiotic prophylaxis and its 2017 blood pressure guidelines
  • The American Society of Anesthesiologists’ physical status classes, amended in October 2025 with the core definitions unchanged
  • The International Association of Dental Traumatology’s 2020 guidelines on dental injuries
  • No “film” for radiographs, so no darkroom or chemical processing questions
  • Smoking as cigarettes per day, or pack-years with one pack counted as 20 cigarettes

The 2027 NBDHE Candidate Guide

The JCNDE said its 2027 Candidate Guides would go online in December 2026, with the final 2027 fees. At its June 24, 2026 meeting it proposed keeping fees at 2026 levels. If you test in 2027, use the 2027 guide, not this one.

Further out, the JCNDE has directed staff to bring in multi-stage adaptive testing for the NBDHE no sooner than 2028. Its April 2026 roadmap says that will shorten the exam and tighten test security. A dental hygiene practice analysis is planned for 2029, to inform later content updates.
