Hawaii driver's license replacement: lost, stolen or damaged

On Oahu you can order an exact copy online for $7 and get it by mail within five weeks, or go to a driver licensing center and pay $6. Kauai's duplicate page describes applying in person, and its fee chart lists $10, as of October 2026. A Hawaii driver's license replacement is also how you get a new address or name printed on the card.

Can you get a Hawaii driver's license replacement online?

On Oahu, yes. Honolulu's online duplicate site sends an exact replacement, with the same name, address and photo, for $7: the $6 duplicate fee plus $1 for the online service. Honolulu says to use it only if you won't need to drive while you wait, because the card is mailed within five weeks. Kauai's main DMV page links to the same portal, but its duplicate page describes only applying in person.

Replacing it in person or by mail

On Oahu, book AlohaQ at a driver licensing center, or at one of the four satellite city halls that do licenses if you're a U.S. citizen or green card holder. You take an eye test, give your fingerprint, pay $6 and have your photo taken. By mail, send a signed letter with your name, the last four digits of your Social Security number or your license number, your date of birth, email and the reason, with a $6 certified check or money order to the City and County of Honolulu. Mailed cards go to Hawaii addresses only.

On Kauai, bring the application marked for a duplicate and an original or certified ID document, such as a birth certificate, Social Security card, state ID, passport, military ID, Medicare card or immigration papers. Photocopies aren't accepted.

Changing your address

Both counties ask you to report a new address within 30 days. On Oahu, send the signed Notice of Change of Address, form CS-L(DL)89, with two proofs of your new address, by email, fax to (808) 768-9096 or mail. That updates your record, not your card, which stays valid until it expires. To get the new address printed, apply for a $6 duplicate. Kauai works the same way: an address change alone doesn't produce a new license, so book a duplicate if you want one.

Changing your name, or replacing a permit

A name change needs an in-person visit. On Oahu, book AlohaQ, fill in the application, bring proof of the change, such as a marriage certificate or court order, and go through the eye test, fingerprint, fee and photo. A lost or damaged instruction permit is replaced the same way as a license: $6 in person on Oahu or $7 online.
