How the 200 scored questions split
Both versions have four categories: Patient Care, Safety, Image Production and Procedures. Here are the eight sections inside them:
| Section | 2022 | 2027 |
|---|---|---|
| Patient interactions and management | 33 | 31 |
| Radiation physics and radiobiology | 21 | 21 |
| Radiation protection | 29 | 25 |
| Image acquisition and evaluation | 26 | 26 |
| Equipment operation and QA | 25 | 25 |
| Head, spine and pelvis | 18 | 20 |
| Thorax and abdomen | 20 | 22 |
| Extremities | 28 | 30 |
The 2022 column is for exams through February 28, 2027 and the 2027 column for exams from March 1, 2027. QA is quality assurance. By category, Patient Care goes from 33 to 31 questions, Safety from 50 to 46, Image Production stays at 51 and Procedures goes from 66 to 72. SI units (the International System of Units) are the main units of radiation measurement in both versions.
Which version of the ARRT exam study guide do you need?
Go by the day you sit the exam, not the day you apply. Your exam window lasts 365 days, so a window that opens in 2026 can run past February 28, 2027. If you might test on or after March 1, 2027, study from the 2027 version.
The same advice applies to retakes. ARRT’s handbook says it updates its content specifications regularly and asks you to compare the version on its website with the one you used before, because you might face content that’s new to you.
What the outline lists, in brief
The pages after the table break each section into topics. In the 2027 version:
- Patient care covers ethics and law, communication, patient transfer, emergencies, infection control and contrast media
- Radiation physics runs from X-ray production to photon interactions and the biological effects of radiation
- Radiation protection covers exposure factors, beam restriction and protecting staff with time, distance and shielding
- Image production covers technique charts, automatic exposure control, digital imaging, picture archiving and communication systems (PACS), Digital Imaging and Communications in Medicine (DICOM) and equipment quality control
- Each procedures section tests positioning, anatomy, adapting the procedure and judging the image, region by region
Each topic in the outline is linked to tasks in ARRT’s Task Inventory for Radiography, which lists the job duties of an entry-level radiographer. ARRT says the document isn’t a curriculum guide: programs teach more than the outline covers.
The ARRT handbook and other documents to download
ARRT publishes several documents for Radiography, each in a version through February 28, 2027 and one effective March 1, 2027. The ARRT handbook for first-time candidates is the 2026 Primary Eligibility Pathway Handbook. It covers:
- Education and ethics requirements, with the three ethics questions on the application
- How to apply, deadlines, ID rules and name changes
- Scheduling, rescheduling and the exam window
- The exam format, the length of each discipline’s exam and test center rules
- Score reports, passing requirements and appeals
The Didactic and Clinical Competency Requirements explain the coursework and clinical procedures you need to complete, and the Task Inventory lists the job responsibilities the exam is built on. Both are on ARRT’s Radiography page.
