CT DMV Form R229 and the documents for your permit test

Bring Form R229 filled in and signed, plus original identity documents. The CT DMV Form R229 (R-229 on the form itself) is the Application for Non-Commercial Learner's Permit and/or Driver's License, and you can print it after you book. You'll need it again at the road test. Photocopies of ID documents aren't accepted.

When do you need CT DMV Form R229?

  • At your knowledge test appointment, completed and signed.
  • At the road test, completed. If you're under 18, your qualified trainer signs to confirm your training.
  • For a motorcycle learner's permit, together with your license.
  • A commercial learner's permit uses a different form, R-229a.

Identity documents

The DMV's checklist (DI-3, revised June 2026) asks for originals or certified copies, all unexpired. You must cover:

  • Two identity documents, at least one from the primary list, like a U.S. birth certificate, a U.S. passport or a permanent resident card. The second can be a Social Security card, a certified school transcript or another item on the list.
  • Your Social Security number, which the DMV checks with the Social Security Administration.
  • Two proofs of Connecticut residency from different sources, showing your name and address and usually dated within 90 days. For a minor, a parent or guardian at the same address can provide them.
  • If you weren't born in the U.S., proof of legal presence, such as an I-94 or permanent resident card.
  • Name change papers, if your name differs from your primary document.

Documents in another language need a translation by a DMV-approved translator. Notarized photocopies and damaged documents aren't accepted.

Everything else to bring to the knowledge test

  1. Print your Connecticut Work Zone Safety Course certificate.
  2. Under 18, bring a parent or guardian, or a notarized Form 2-D (Certificate of Parental Consent) if they can't come.
  3. Bring your Connecticut non-driver ID, if you have one. You hand it in when you get the permit.
  4. Bring glasses or contacts, if you need them to pass the vision test.

The $40 testing fee and $19 permit fee are paid online when you book.
