What to bring for a Nevada permit, license or ID

For a Nevada permit, license or ID card, you bring one proof of identity, proof of your Social Security number, two documents showing your Nevada home address, and proof of any name change. They must be originals or certified copies. The same list works for a REAL ID, the standard card and the Driver Authorization Card, but each accepts different identity documents.

Proof of identity

For a REAL ID, bring one of these: a U.S. passport or passport card, an original or certified U.S. birth certificate, a Consular Report of Birth Abroad (FS-240), a naturalization or citizenship certificate (N-550 or N-560), a Permanent Resident Card (I-551), a foreign passport with the right visa and I-94, or an Employment Authorization Card (I-766).

Hospital birth certificates, foreign licenses, foreign birth certificates, military IDs and consular IDs don't work for a REAL ID or standard card. Several of them do count toward a Driver Authorization Card.

Social Security number

Bring your Social Security card, a W-2, an IRS 1099 or a printed pay stub showing your full number and current legal name. If you've applied for a number but don't have it yet, you can say so on your application instead.

Nevada residency requirements: two proofs of address

  • Dated within 60 days: a rent receipt or lease, a utility bill, a bank or credit card statement, a pay stub, a Nevada school record, a voter registration card, or a hotel or RV park record for a stay of 30 days or more.
  • Most recent copy: car insurance card or bill, a medical bill, a tax or property tax record, or a mortgage or deed of trust.
  • Printouts from online accounts are accepted.
  • No documents in your name? The DMV's own forms can help: Certification of Nevada Residency (DMV 005), Relief Agency or Shelter Certification (DMV 115) and Property Owner Residency Affidavit (DMV 116).

Nevada REAL ID

A Nevada REAL ID has a gold Nevada outline with a star. You need one, or another accepted ID such as a passport, to board a domestic flight or enter secure federal buildings. A standard Nevada license still lets you drive and is fine for state uses, but it's marked 'Not for REAL ID Purposes'.

You must apply in person with an appointment. A REAL ID must show your physical address. The upgrade is $8.50 for a license or ID, or nothing if you add it during another transaction, as of October 2026.

Under 18 or changing your name

Teens applying for a permit also bring a Certification of Attendance (DMV 301) signed within the last 60 days, the Minor Affidavit (DP-38), and a parent or guardian.

If your name has changed, bring proof of every change, such as a marriage certificate, divorce decree, adoption record or court order. A Social Security card doesn't count as proof of a name change.
