Vermont DMV forms for permits, licenses and IDs

For a learner's permit, a license, a mail-in renewal or an address change, the form is VL-021, the License/Permit Application. Vermont DMV forms are free PDFs on the DMV's forms page, and the DMV says most can be filled in with Adobe Reader. The application, the non-driver ID form and the minor residency form also come in 14 other languages.

Where the forms go

The forms page says completed forms and fees are generally mailed to the DMV at 120 State Street, Montpelier, VT 05603-0001. A first non-driver ID and a first enhanced license are exceptions: you apply for those in person at an office. For the permit itself, you take the test online, and the application has to be signed by a parent or guardian first if you're under 18.
