Which Nebraska DMV forms do you need for a permit?
| Form | What it’s for |
|---|---|
| Data form (English) | New, renewal or replacement permit, Class O or Class M license, or State ID card |
| Data form (Spanish) | The same application, in Spanish |
| Out of State Renewal Data Form | Renewing or replacing by mail while you’re outside Nebraska |
| Document Verification Requirements | The documents that prove identity, address and name changes |
| Social Security Verification Form | Proof of your Social Security number |
| Statement of Physician and Vision Statement | When DMV asks for a doctor’s or eye doctor’s report |
Forms for young drivers
Applicants for a school permit (SCP) or provisional operator’s permit (POP) who don’t take a driver safety course need the first two forms.
| Form | What it’s for |
|---|---|
| SCP/POP 50 Hour Certification | Signed by a parent, guardian or licensed driver 21 or older, instead of a driver safety course |
| SCP/POP Log | Tracking the 50 hours, including the 10 at night |
| Under Age 18 Certification | A parent’s statement of a child’s identity when there’s no certified birth certificate |
Nebraska DMV online services
Some tasks don’t need a form at all. If you qualify, dmv.nebraska.gov lets you get a school permit, provisional operator’s permit (POP) or Class O license, renew a license or ID card, replace a lost card, change your address and buy your driving record. Non-US citizens can’t use the online license services.
