How do you replace a lost driver’s license in Nebraska?
- A completed data form, or fill one in at the office
- One proof of identification showing your name and date of birth
- Two proofs of address, if you’ve moved or your address hasn’t been checked before
- The fee, and you may need to meet the vision and medical rules
Expect a new photo at every visit. You leave with a 30-day receipt that lets you drive, and DMV says the card is mailed within 14 business days; call Driver Licensing Services if it hasn’t come in 20 days. Once a replacement is issued, the old card is void, even if you find it.
If you’re out of state
You can apply by mail with DMV’s out-of-state data form, a proof of identification and two proofs of address. Contact Driver Licensing Services in Lincoln before you mail anything, to confirm the fee and how to pay. Permits, commercial licenses and commercial permits can’t be replaced this way and need an office visit in Nebraska.
Name and address changes
State law gives you 60 days after a change of name or address to apply for a replacement. For a name change, bring a certified marriage license, divorce decree or court order that links your old and new names. A change of address alone can be done online.
