NBCOT exam: the OTR and COTA certification exams

The National Board for Certification in Occupational Therapy (NBCOT) gives two exams: one for the Occupational Therapist Registered (OTR) credential and one for the Certified Occupational Therapy Assistant (COTA) credential. You take the NBCOT exam on a computer at a Pearson VUE test center, on a day you choose, and both versions run 4 hours. Results come out on NBCOT’s set score dates, and a scaled score of 450 passes either exam.

How it works

How the NBCOT exam works: one 4-hour block, two item types

Scaled score, OTR and COTA
300 to 449: fail450 to 600: pass
  1. You take the exam on a computer at a Pearson VUE test center, any day of the week the center is open, once NBCOT has emailed your Authorization to Test (ATT) letter.
  2. Both exams use single-response multiple choice items, with three or four options and one correct answer.
  3. The OTR exam adds multi-select scenario sets: an opening scene, then four items, each with six options of which you pick the three best.
  4. The COTA exam adds six-option multi-select items on their own, where you also pick three.
  5. You get credit for each correct option you pick, and nothing is taken off for a wrong one.
  6. Some items are unscored field-test items, mixed in at random so you can’t tell which they are.
  7. You can flag items, change answers, highlight, strike out options, change colors and zoom up to 200% while time allows. Breaks are allowed, but the clock keeps running.

  3. Choose a Pearson Professional Center or Pearson VUE Test Center if you need the earplugs or noise-reducing headphones they provide.

FAQ

Common questions

What is the NBCOT exam?

It’s the national certification exam for occupational therapy in the US, in two versions: OTR for occupational therapists and COTA for occupational therapy assistants. NBCOT says its main purpose is to protect the public by certifying only candidates who have the knowledge needed to practice, and each exam measures entry-level competence. Passing gives you the credential, but your state license is a separate step with your state board.

How long is the NBCOT exam?

Both exams give you four hours of exam time. Plan on about 4 hours 45 minutes at the center, because check-in, the optional tutorial, a survey and check-out come on top. The tutorial doesn’t use your exam time, but you must accept the acknowledgment within seven minutes or the exam closes and you lose the appointment.

What’s the difference between the OTR and COTA exams?

The OTR exam has 180 items across four domains, and the COTA exam has 190 items across three. Only the OTR exam uses scenario sets. You take the one that matches your degree: an occupational therapy program for the OTR, an occupational therapy assistant program for the COTA.

Can I take the NBCOT exam online?

No. NBCOT describes a standard exam as a timed, four-hour, computer-based exam taken in person at a test center, and every place it lists for taking it is a Pearson test center. What you do online is apply, book your Pearson appointment, use NBCOT’s study tools and see your result, all through MyNBCOT.

What does NBCOT stand for?

The National Board for Certification in Occupational Therapy, the national certification body for occupational therapy professionals in the US. Its OTR and COTA certification programs are accredited by the National Commission for Certifying Agencies (NCCA). NBCOT handles initial certification and renewal; state boards issue licenses.

Is NBCOT the same as AOTA?

No. NBCOT writes and runs the certification exams. The American Occupational Therapy Association (AOTA) is the national professional association for occupational therapy practitioners and students, a separate organization. NBCOT says it doesn’t require, approve or endorse any prep course or study material, whoever sells it.
