Louisiana driver’s license requirements: the documents to bring

For a first license, permit or TIP you show two primary documents, or one primary and two secondary documents, plus one proof of your Louisiana address. Every document must be an original or a certified copy, because the OMV turns away photocopies. Teens also need a parent or guardian there in person, and the Louisiana driver’s license requirements for a REAL ID add a second proof of address.

  • 2Primary IDs, or 1 primary and 2 secondary
  • 1Proof of address, or 2 for a REAL ID
  • 0Photocopies accepted

What are the Louisiana driver’s license requirements for ID?

OMV policy 6.00 lists the documents it accepts. Primary documents include:

  • A U.S. birth certificate, original or certified
  • A U.S. passport book or passport card. Under 18, you also bring a certified birth certificate or custody papers
  • A certificate of naturalization or citizenship
  • A permanent resident card (I-551), or a foreign passport with the right immigration papers
  • A current U.S. military ID or a DD-214

Secondary documents include a Social Security card, a Louisiana school or college photo ID, an employer photo ID, a high school diploma, a health insurance card, a vehicle title or registration in your name, and a U.S. marriage certificate. A Social Security number given out loud doesn’t count as a secondary document, though you must still give the OMV your number.

Proof of your Louisiana address

You bring one original document with your name and Louisiana home address. The OMV’s list includes:

  • A utility bill for your home, such as water, electric, internet or phone
  • A bank, credit card or loan statement
  • A pay stub, a W-2 or a letter from your employer
  • A school record, transcript or report card
  • A current lease or rent receipts that cover today’s date
  • The Louisiana license or ID of a parent, guardian or spouse who lives with you, if they come with you

You can black out account numbers and money figures on these papers.

If you’re under 18

  • Your custodial parent or legal guardian comes with you and signs the application.
  • You and the parent or guardian both show ID.
  • A guardian brings the papers that show the guardianship. With joint custody, only the domiciliary parent may sign.
  • When you swap a TIP for a permit or license, you bring the sealed envelope from your driving school with your Certificate of Completion.

Extra papers for a REAL ID

A REAL ID needs proof of identity and lawful status, your Social Security number and two proofs of address from separate sources. It costs the same as a standard card. You can’t get your first REAL ID online: you visit an OMV office or a Public Tag Agent.

OMV forms you may need

The OMV posts its driver’s license forms online, among them the Application for License or Identification Card, the Vision Examination Form (DPSMV 2301) and the Medical Examination Form (DPSMV 2302), which the OMV can ask a doctor or eye specialist to fill in for you.
