Which documents are needed for a Utah driver's license?
For U.S. citizens, U.S. nationals and permanent residents, bring one from each group.
- Identity and lawful status: a valid U.S. passport or passport card, a certified birth certificate from a state vital records office, a Consular Report of Birth Abroad, a Permanent Resident Card, or a Certificate of Naturalization or Citizenship.
- Social Security number: your signed Social Security card. If you don't have it, a W-2, SSA-1099, non-SSA-1099 or pay stub showing your name and full number.
- Two proofs of Utah address, showing your name and dated within 90 days: a bank statement, court documents, a mortgage or rental contract, a major credit card bill, a property tax notice, a school transcript, a utility bill or a vehicle title.
- For a first license, proof of driving experience: a driver education completion if you're under 19, or a license from another state or country.
What isn't accepted
Photocopies, faxes, laminated birth certificates, small wallet-size birth certificates and hospital birth certificates are all turned away. Documents in another language need a certified English translation.
If your name doesn't match your birth certificate or passport, bring the legal proof, such as a certified marriage certificate, divorce decree or court order. The office scans your documents and gives them straight back.
If you're not a U.S. citizen
With verified lawful presence, you apply for a limited-term license, using an unexpired Employment Authorization Document, or a foreign passport with a visa and I-94, plus a federal document showing your status. If you can't show lawful presence, you may qualify for a driving privilege card, which has its own list, including an ITIN and a fingerprint card.
Before you go
Check your papers against the division's list the night before. Approval can take more than a day if something needs checking, and you may have to come back. Bring your appointment confirmation number too.
