Lost your Utah driver's license? How to replace it

If you've lost your Utah driver's license, or it was stolen, book a duplicate license appointment with the Driver License Division. At the office you take an eye test, get a new photo and pay $23, as of October 2026. There's no test. You leave with a temporary license, and the new card arrives in about 4 to 6 weeks.

How do you replace a lost Utah driver's license?

  1. Book a duplicate license appointment online.
  2. Bring at least one document from the division's required documents list, since you don't have your license. Bring a police report if you filed one.
  3. At the office, take the eye test, have your photo taken and pay $23.
  4. Take your temporary license home. The card is mailed in about 4 to 6 weeks.

Replace or renew?

A replacement keeps the same expiration date as the license it replaces. If your license expires within six months, renew it instead, so you're not paying twice.

Changing the name on your Utah driver's license

A name change is done in person, as a replacement. Bring your current license and the legal proof of your new name, such as a certified marriage certificate, divorce decree, updated passport, amended birth certificate or court papers. Foreign documents need a certified English translation.

Changing your address

Utah law says you must tell the division within 10 days of moving, and you can do that online. The post office won't forward the division's mail. If you want the new address printed on your license, book a replacement and bring two proofs of address, or update it online first and skip that step.
